Before you begin
- 1Complete all required fields before arrival at the Port Health screening counter.
- 2Declare recent travel, symptoms, exposure, and contact history truthfully.
- 3Present your reference number to the Port Health Officer when requested.
- 4A parent or guardian should complete the declaration for children under 12 years.
- 5Keep your reference for possible Ghana Health Service follow-up within 21 days.
Providing false information or refusing to comply with Port Health instructions may trigger measures under Ghana's Public Health Act 2012 (Act 851).
